More than 20 million people in the United States use electronic cigarettes or e-cigs as an alternative to tobacco smoking. While many users believe "vaping" is a healthier alternative to regular cigarettes, recent research into the health effects of e-cigs could put a damper on that belief. There's particular concern among dentists that this popular habit could harm users' dental health.
E-cigs are made with a chamber that holds the liquid vaping solution and a heating mechanism to heat the liquid and vaporize it. Users inhale the vapor, which contains nicotine and flavorings, as they would a traditional cigarette.
The nicotine alone can be problematic for dental health as we'll see in a moment. But the vapor also contains aerosols that some research indicates could damage the inner skin linings of the mouth in a similar fashion to the smoke of traditional cigarettes. One study by researchers with the Université Laval in Quebec, Canada found evidence that e-cig vapor increased the death rate of mouth cells, and led to greater cell irregularities over time.
According to other studies, there's evidence that e-cig vapor may disrupt the balance of the oral microbiome, the communities of both beneficial and harmful bacteria that normally live in the mouth. The imbalance in favor of more harmful bacteria could increase the risk for dental disease, particularly periodontal (gum) disease.
Finally, nicotine from e-cigs seemed to create similar conditions in the mouth as it does with tobacco. Nicotine in any form can constrict blood vessels and reduce the body's ability to fight infection and to heal. Research indicates both forms of nicotine increase the risk for dental disease and make treatment more difficult.
These findings only identify conditions created by e-cigs that could be problematic for future dental health. Although we don't fully understand the long-term health effects of this new habit, based on the evidence so far the mouth may not fare so well. It's looking like e-cigs may be no safer for your teeth and gums than the cigarettes they replace.
If your dentist found tooth decay on your last visit, you might have been surprised. But tooth decay doesn't occur suddenly—it's a process that takes time to unfold.
It begins with bacteria—too many, that is. Bacteria naturally live in the mouth, but when their populations grow (often because of an abundance of leftover sugar to feed on) they produce high amounts of acid, a byproduct of their digestion. Too much acid contact over time softens and eventually erodes tooth enamel, making decay easier to advance into the tooth.
So, one important strategy for preventing tooth decay is to keep your mouth's bacterial population under control. To do that, here are 4 common-sense tactics you should perform between dental visits.
Practice daily hygiene. Bacteria thrive in dental plaque, a thin film of food particles that builds up on teeth. By both brushing and flossing you can reduce plaque buildup and in turn reduce disease-causing bacteria. In addition, brushing with a fluoride toothpaste can also help strengthen tooth enamel against acid attacks.
Cut back on sugar. Reducing how much sugar you eat—and how often –deprives bacteria of a prime food source. Constant snacking throughout the day on sweets worsens the problem because it prevents saliva, the body's natural acid neutralizer, from reducing high acid levels produced while eating. Constant snacking doesn't allow saliva to complete this process, which normally takes about thirty minutes to an hour. To avoid this scenario, limit any sweets you eat to mealtimes only.
Wait to brush after eating. Although this sounds counterintuitive, your tooth enamel is in a softened state until saliva completes the acid neutralizing process previously described. If you brush immediately after eating you could brush away tiny particles of softened enamel. Instead, rinse your mouth out with water and wait an hour for saliva to do its work before brushing.
Boost your saliva. Inadequate saliva flow could inhibit the fluid's ability to adequately neutralize acid or provide other restorative benefits to tooth enamel. You can improve flow with supplements or medications, or by drinking more water during the day. Products with xylitol, a natural sugar alternative, could give you a double benefit: chewing gums and mints containing it could stimulate more saliva flow and the xylitol itself can inhibit bacterial growth.
February is National Children's Dental Health month, sponsored annually by the American Dental Association. As important as good oral health is to a child's overall health and development, tooth decay tops the list as the most common chronic childhood disease. In fact, over 40% of children ages 2-11 have had cavities in their baby teeth.
If unchecked, tooth decay can have a profound impact on a child's quality of life. The good news is that tooth decay is preventable, and often reversible if detected early. Here are some things you can do to set your child on the path to good dental health for life:
Get your child in the habit of brushing and flossing every day. Cavity prevention starts at home, so teach your child to brush twice a day with fluoride toothpaste—but use only a smear of toothpaste the size of a grain of rice before age 3, and a pea-sized amount from ages 3-6. Introduce dental floss into the routine when you notice that your child's teeth are starting to fit closely together. Children generally need help brushing until age 6 or 7 and flossing until around age 10.
Encourage tooth-healthy eating habits. Provide your child with a balanced diet with plenty of vegetables, fruits and whole grains. Stay away from sugary snacks and beverages, especially between meals. If children drink juice, they should do so with meals rather than sipping juice throughout the day or at bedtime. Even 100% juice has natural sugars and can be acidic, which can harm teeth with prolonged exposure.
Establish a dental home early. Tooth decay isn't always easy to spot with the naked eye, so regular dental visits should start no later than a baby's first birthday. We can check the development of your child's teeth and spot any issues of concern. The earlier tooth decay is caught, the less damage it can do. Even if there are no dental problems, establishing a dental home early on will help your little one feel comfortable at the dental office.
Ask about preventive dental treatments. Fluoride varnishes or rinses are frequently recommended to help prevent cavities, particularly for children at higher risk of getting cavities. Dental sealants, another preventive treatment, are a coating commonly applied to molars to seal out tooth decay. According to the U.S. Centers for Disease Control, children ages 6-11 with dental sealants have nearly three times fewer cavities than children who do not have sealants.
The key to healthy smiles for life is to start your child at a young age with good habits at home and regular dental visits. Periodontal (gum) disease is as common as it is destructive. Almost half of all adults 30 and older have some form—and those numbers increase to nearly three-quarters by age 65.
Fortunately, we have effective ways to treat this bacterial infection, especially if we catch it early. By thoroughly removing all plaque, the disease-causing, bacterial biofilm that accumulates on tooth surfaces, we can stop the infection and help the gums return to normal.
Unfortunately, though, you're at a greater risk for a repeat infection if you've already had gum disease. To lower your chances of future occurrences, we'll need to take your regular dental exams and cleanings to another level.
Although everyone benefits from routine dental care, if you've had gum disease you may see these and other changes in your normal dental visits.
More frequent visits. For most people, the frequency norm between dental cleanings and exams is about six months. But we may recommend more visits for you as a former gum disease patient: depending on the advancement of your disease, we might see you every three months once you've completed your initial treatment, and if your treatment required a periodontist, we may alternate maintenance appointments every three months.
Other treatments and medications. To control any increases in disease-causing bacteria, dentists may prescribe on-going medications or anti-bacterial applications. If you're on medication, we'll use your regular dental visits to monitor how well they're doing and modify your prescriptions as needed.
Long-term planning. Both dentist and patient must keep an eye out for the ongoing threat of another gum infection. It's helpful then to develop a plan for maintaining periodontal health and then revisiting and updating that plan as necessary. It may also be beneficial to perform certain procedures on the teeth and gums to make it easier to keep them clean in the future.
While everyone should take their oral health seriously, there's even greater reason to increase your vigilance if you've already had gum disease. With a little extra care, you can greatly reduce your chances of another bout with this destructive and aggressive disease.

It’s been a long road with your braces, but now they’re finally off. Hopefully the first glimpse of your new smile more than made up for the time and effort they required.
But while braces removal is a big milestone, it’s not the end of your treatment—not, that is, if you want to keep that new smile! You’ll now need to wear an appliance called a retainer for a few years or, in some cases, from now on.
Orthodontic retainers are a must after braces for the same reason braces work in the first place—your teeth can move. While the teeth attach to the jawbone via the roots, they’re firmly held in place by an elastic gum tissue network called the periodontal ligament. This tough but elastic tissue lies between the teeth and gums and attaches securely to both with tiny fibers.
While the ligament provides stability, it’s also dynamic—constantly remodeling to allow the teeth to move in response to biting pressure and other mouth factors. Orthodontists use this mechanism when moving teeth to better positions. The braces apply pressure on the teeth in the desired direction and the periodontal ligament responds as the teeth move.
Afterward, however, the ligament can still retain a kind of “muscle memory” for a time of the teeth’s old positions. Free of the pressure once supplied by the braces the teeth have a tendency, especially early on, to “rebound” to where they were.
A retainer helps prevent this by exerting just enough pressure to “retain” the teeth in their new positions. In the beginning this may require wearing the appliance around the clock, but you may be able later to reduce wear time to just a few hours a day. Rebounding is unpredictable, so you should continue to follow your orthodontist’s recommendations on retainer wear.
Wearing a retainer may seem like a drag, but it’s absolutely essential. Being diligent about it will help ensure that the beautiful smile you and your orthodontist worked so hard to obtain stays with you for years to come.
